As an innovative, youth-driven program, we educate, engage, and empower youth to help create positive change. We work directly with young people to ensure youth voice is included in decisions that affect young people, to improve adolescent health care quality, and to improve the overall health and well-being of their generation.
The Core PATCH Package provides communities an innovative, positive youth development approach to improve adolescent patient-provider communication, health care experiences, and health care transition efforts. PATCH Sites are coached through hiring, training, and supervising a team of Teen Educators who facilitate two complementary workshops–one for health care professionals and the other for peers.
The PATCH Youth Advocacy Fellowship offers a framework integrating youth voice into adolescent health related program and policy decisions at the local and state level while supporting youth to grow as leaders and activists. The Fellowship is currently being implemented in Wisconsin only.
The PATCH for Teens and PATCH for Parents Toolkits are resources for all types of educators who want to help young people gain more independence and responsibility over their health and health care decisions. Each Toolkit contains a step-by-step guide that provides information, materials, and activities necessary to deliver an empowering and interactive health education session.
PATCH® offers various adolescent health care resources (brochures, forms, posters, materials) for youth, health care professionals, parents, guardians, and caregivers. These resources are available in both English and Spanish in the PATCH Store and are great for use in classrooms, clinic waiting areas, youth centers, and other community spaces.
We have seen firsthand how the stories, experiences, perspectives, and insights of young people help spark productive conversations, fresh thinking, and initiate bold and lasting change.
